Step 1
Developing YOUR Care Profile
When you call Southern Angels Home Care, we start by gathering information on how we can best help you. We will ask questions to get a better understanding of your wishes. Then, we will arrange an in-home visit to perform a basic needs and environment assessment as well as answer any questions you may have.
Step 2
Creating a Patient-Center Care Plan
Next step, is to create a patient-centered care plan that is tailored to YOUR needs. This plan will incorporate you and your loved ones history, routines, and preferences in order to create support of your activities that will provide the most benefit. We will review this plan with you in detail and modify it as YOU see fit.
Step 3
Identifying the Best Caregiver for YOU
Southern Angels Home Care understands allowing someone in your home is a BIG step. We are honored and grateful that you put your trust in us. Which is why it is important we make the perfect match of patient and caregiver. There are numerous factors that go into creating the best match. All caregivers are screened, including background, drug-screen, and reference checks. YOUR comfort matters the most, so if you ever have a concern we will work to find you a new caregiver.

Providing Ongoing Service
Southern Angels Home Care keeps detailed notes so YOU know exactly what happened each time caregivers provide YOU service. Supervisory staff also make periodic visits to your loved one's home to ensure all care provided is going as YOU wish. That is because YOUR care team is trained to notice and report changes in health or other conditions, so we can quickly adjust YOUR care if it is needed.
